Beginning August 25, 2016, the Purdue IRB online submission system was updated in response to user comments. Many online questionnaires have been removed, and have been replaced with fillable pdfs that the PI must download from the Forms tab, complete, and upload as attachments into CoeusLite. The new pdf forms are: Exempt Research Request, Amendment to Approved Study, Personnel Amendment Table, Continuing Review or Closure Report, Continuing Review Form: Key Personnel Table continued (for use when there is not enough space on the Continuing Review Report.)

Please replace forms that you have saved with the new forms.

Mac Users: Do not use Preview when opening and completing the PDF forms. Adobe Acrobat or Acrobat Reader must be used or the IRB will not be able to view your work.

All Users: If using Chrome, please save the PDF before filling it out. Edits made to the PDF in the browser preview will not save.
