Notice: IRB submissions must be completed through the Purdue online CoeusLite system. See manuals below. To receive information about upcoming trainings, email

Beginning August 25, 2016, the Purdue IRB online submission system was updated in response to user comments. Many online questionnaires have been removed, and have been replaced with fillable pdfs that the PI must download from the Forms tab, complete, and upload as attachments into CoeusLite. The new pdf forms are: Exempt Research Request, Amendment to Approved Study, Personnel Amendment Table, Continuing Review or Closure Report, Continuing Review Form: Key Personnel Table continued (for use when there is not enough space on the Continuing Review Report.)

Please replace forms that you have saved with the new forms.
